Asante Transfer Admit Center

Asante TAC streamlines admissions from Emergency Departments, outside referring physician offices and other hospitals, or health care facilities by providing dedicated resources to coordinate the services related to patient admission or transfer.

We are dedicated to getting patients to the care they need at Asante.

Services provided

• Facilitate internal and external transfers or admissions.
• Ensure that all special patient needs are appropriately considered.
• Provide smooth transitions for patients. 
• Work collaboratively with nursing, providers and other disciplines to coordinate admissions and transfers.
• Understand complex medical situations that require the coordination of specialty services and work to facilitate meeting all patient’s needs.

The Transfer Admit Center process

1. A physician, clinical care provider or hospital representative calls the Asante Transfer Admit Center to admit or transfer a patient to an Asante hospital location.
2. TAC staff facilitates a conversation between the referring and accepting providers to determine the appropriate site of service related to the patient’s condition and needs.
3. TAC staff work with the referring facility to understand the patient’s clinical presentation and pertinent patient history. Once a patient has been accepted to the appropriate Asante hospital, the Asante TAC will assign the appropriate inpatient hospital bed. The referring and receiving facility (including providers) are updated on the acceptance status of the patient.
