What is MyChart?

Patients of Asante and members of our Community Connect program can now securely access their personal medical record online, from the privacy of their home or any other location with an internet connection.

Did you know that MyChart members can:

Access your health records securely, when you need them.

  • Go online to view their current health issues.
  • View wait times and save your spot in line at most imaging and lab locations.
  • Start a Video Visit on your mobile device.
  • Get an estimate for upcoming services (no account needed).
  • Use preCheck-In to save time at your appointment.
  • View details of past appointments.
  • Request renewals of prescriptions.
  • Request or schedule appointments.
  • Check results of lab and imaging tests.
  • See statements and make online payments.
  • Send questions or requests to providers.

How to sign up

Existing patients may use an activation code to sign up for a MyChart account. This code allows you to create your new MyChart user ID and password, which you will then use to log on to MyChart. All patients over age 18 will receive an activation code at the end of each Asante visit, so you can sign up at home when it's convenient for you. If you need assistance getting your access code, please contact us.

Patients under 18 (and/or their parent/legal guardian) must speak with their doctor in person before signing up for MyChart.

New patients do not need an activation code to sign up for a MyChart account. Our self sign up process enables new patients to create an account to access telemedicine services without having to schedule an in-person office visit with Asante.

Accessing your medical records just got easier

Asante is introducing new MyChart features that will make sharing your electronic health records easier while helping you better manage your health.
Whether you're receiving care at multiple places locally, or you're traveling and need to share your medical information on the spot, you can access these features through your MyChart account.

MyChart Central

Access all your records in one convenient place

MyChart CentralIf you have two or more MyChart accounts from other health care providers, MyChart Central allows you to access all those accounts with a single username and password. You can also save your health records to a thumb drive and take them wherever you go.

To sign up, log into your MyChart account and locate the Health icon, then click on "Lucy." On that page, click on the "Link Med Up with Lucy and MyChartCentral" button. Click on "Create a New Account" to complete the signup process.

Customize your personal health information

MyChart Central also provides you with access to Lucy. Lucy is a personal health record that gives you a permanent home to organize all your medical information. You can request an updated copy of your medical record at any time and store it in Lucy. You can also add personal information about your health, including medications, allergies, health issues and immunizations, and share it wherever you receive care.

Lucy is available as part of MyChart Central and can be accessed at www.mychartcentral.com after you have created a MyChart Central account. You can also access Lucy through your regular MyChart account through the Health icon.

Like your MyChart account, these features are password-protected and delivered via an encrypted connection to keep your health information safe from unauthorized users.
